Daily Behavior Form


Intervention

This intervention holds students accountable on a daily basis, and promotes self responsibility. It is a chart where the student can rate how well they did in specific behaviors throughout the day.




How it applies to Tier 2

  • Low cost to teachers and function based.

  • Specific to the student can be adjusted to their behavioral needs.

  • Is Available until teacher deems fit.




How to use the Intervention

  • Decide the main Behaviors you want to implement onto their behavior chart.

  • Include a rating system on their behavior chart, it can be with numbers or with visuals.

  • Have the student fill out their behavior chart and at the end of the day go over it with them.

  • make corrections on their ratings where you think they are needed (in a different color)

  • Send behavior chart home with the student and have them return it to school the next day signed by a parent or guardian.
